To Nominate a Living Donor for the Hall of Fame...

To nominate a living donor for the Living Donor Hall of Fame, please complete the form below and submit it by clicking on the "Submit Entry" button.

Only people who are living organ or bone marrow donors can be admitted to the Living Donor Hall of Fame. Living donors should feel free to nominate themselves. The hall of fame is not for deceased donors or donor families.

You will receive a confirmation of your nominee's admission to the Living Donor Hall of Fame by email. If you wish, we will also notify the living donor.

9. Optional: Provide the living donor's email address if you would like Living Donors Online! to send a note on your behalf notifying them of their admission to the Living Donor Hall of Fame. Type the living donor's email address in the box, including the domain name, for example, donor_email@aol.com:

10. Optional: If you wish to include a picture of the living donor to display with the Hall of Fame entry, click on the button to browse and choose a file from your computer—maximum file size of 1.5 MB:   

11. Optional: Would you like us to enter the date of donation on the LDO Calendar of Anniversary Dates? This is a special calendar that contains the dates on which people donated so LDO visitors can celebrate the anniversaries of living donors.
